The theme for 2022 is A Person’s Journey and this session focused on patient engagement and recall. Objectives of this Transformation Collaborative included:
- Sharing challenges, opportunities, and lessons learned on front desk/check-in workflows, and how those staff members and workflows can support the patient’s journey
- Discussing meaningful integration of front desk and patient access staff in the care team
- Sharing challenges, opportunities, and lessons learned on data collection at the front desk
Each session is thoughtfully designed to be interactive and energizing, equipping participants with information and tools they can share with their teams and quickly put into practice. Sessions are designed to promote integrated care, and members from every service line and department (dental, behavioral health, medical, pharmacy, health information technology, and billing) are welcome and encouraged to participate.
The health centers were engaged, sharing, and learning from one another and they walked away with new solutions and methodologies to try. There was a mix of work, fun, and thinking through actions to take back and implement. The themes and recommended participants of the remaining 2022 sessions are below. We invite all interested health center staff to participate!
