The Transformation Collaborative is a series of multidisciplinary events hosted by the Iowa Primary Care Association, INConcertCare, and IowaHealth+, bringing together health center leaders, clinicians, and team members to share knowledge and develop skills to meet the ever-changing needs of healthcare today.
- Each session is thoughtfully designed to be interactive and energizing, equipping participants with information and tools teams can quickly put into practice.
- Registration is required to attend, and is free and open to all Iowa community health center staff.

2025 Transformation Collaboratives:

- Day 1: IowaHealth+ Quality & Operations Bootcamp
- Session Purpose: Provide comprehensive education on the 2025 HEDIS Value Based Care contract measures.
- Morning session will focus on the measure requirements and how to utilize pre-visit planning, orders, documentation, coding and workflows to achieve outcomes.
- Afternoon session will be divided into breakout sessions focused on quality operations planning and progress monitoring and reporting.
- This education will be relevant for all IH+ centers regardless of EHR.
- Intended Attendees: Quality staff, EHR team members including ESAs, Provider, Content Builders, COOs, Clinical managers/supervisors, team leads, CMOs
- Session Purpose: Provide comprehensive education on the 2025 HEDIS Value Based Care contract measures.
- Day 2: Enhancing Quality through Data and Practice
- Session Purpose: Equip clinical team members with the knowledge and skills necessary to excel in UDS reporting while strengthening clinical competencies.
- Participants will gain an understanding of UDS measures & their importance in quality improvement and practical strategies to integrate them into daily workflows
- This session will combine didactic learning, hands-on exercises, and peer discussions to provide participants with actionable tools to take back to their teams.
- Intended Attendees: Quality staff, clinical managers/supervisors and team members, team leads
- Session Purpose: Equip clinical team members with the knowledge and skills necessary to excel in UDS reporting while strengthening clinical competencies.
