Transformation Collaborative

The Transformation Collaborative is a series of multidisciplinary events hosted by the Iowa Primary Care Association, INConcertCare, and IowaHealth+, bringing together health center leaders, clinicians, and team members to share knowledge and develop skills to meet the ever-changing needs of healthcare today.  Each session is thoughtfully designed to be interactive and energizing, equipping participants with information and tools teams can quickly put into practice. Registration is required to attend, and is free and open to all Iowa community health center staff.

The agenda for the February 2024 Transformation Collaborative session is currently being developed. This Transformation Collaborative will be focused on using DMAIC Way continuous improvement methodologies to create resources for health centers across the network that support better management of patient attribution and assignment at the point of care.

Participants can expect the following topics and activities:

  • Enrollment, Attribution & Outreach Landscape – Why is it important to payors, providers, and patients?
  • Morning Workshop: Using DMAIC Way Tools to map out current state outreach and engagement workflows for attributed patients.
  • Health Center Presentations: Hear from your colleagues how they have put continuous improvement methodologies into practice for this work.
  • Afternoon Workshop: Using DMAIC Way Tools to identify process improvement activities and apply to your health center outreach and engagement workflows.
  • Subject matter experts and consultants from the Iowa Primary Care Association will be available to assist in answering questions and facilitating through the continuous improvement activities.

Who should attend?

The suggested audience for this session includes all health center team members who are involved in patient engagement activities. Examples may include: patient access leaders and staff (registration / front desk), enrollment specialists, membership specialists, Consumer Advisory Committee members, Value-Based Care Task Force members, and all other team members involved in your outreach, enrollment, and scheduling.

The suggested audience information above is meant to provide guidance and clarity around the topics covered.  Health center team members from every service line and department (dental, behavioral health, medical, pharmacy, health information technology, billing, marketing and communications, benefits specialists, Health Equity Champions, providers, care teams, IH+ committees and taskforce, and administration) are welcome and encouraged to participate.
