Rural Health Transformation: Healthy Hometowns

Healthy Hometowns is Iowa's submission to the Rural Health Transformation Program, a federal funding initiative managed by the Centers for Medicare and Medicaid Services (CMS). The bold and comprehensive proposal is aimed at building a high-quality, sustainable system of care that improves the health, well-being, and quality of life in rural communities across the state.

  • Communities of Care Co-Location

    The Communities of Care Co-Location initiative is intended to support new models of collaboration in rural Iowa by encouraging multiple providers and organizations to work together in shared spaces to improve care coordination and reduce barriers to access. Potential co-location models may include combinations of primary care, behavioral health, oral health, nutrition services, school-based services and other community-based supports. The Iowa PCA will share training, tools, and support to help communities plan this work. 

  • School-Based Services

    This project will help rural health care providers, schools, and local partners leverage schools as trusted locations in rural communities for Iowans to access services. This initiative aims to make it easier for children and families living in rural Iowa to get care close to home. Funding will be invested in healthcare providers to collaborate with schools and other partners to offer more comprehensive and coordinated care including physical, mental/behavioral, and dental health, and other services and supports Iowa children and families need to stay or become healthier. The Iowa PCA will share training, tools, and support to help communities plan this work.

The Communities of Care initiative is supported by the Centers for Medicare and Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling approximately $209,040,063.71 with 100 percent funded by CMS/HHS.
