Integrated Health Consultant (Oral Health area of focus)

The Integrated Health Consultant is responsible for developing, evaluating, supporting, and/or expanding integrated health programs and initiatives within the defined focus area(s) for the Iowa PCA and promote a culture of process improvement. The Integrated Health Consultant will development and maintain community and state partnerships and increase awareness of integrated health as an integral component of overall health. Focus areas for integrated health consultants include, but are not limited to, medical, oral health, behavioral (brain) health, supportive services, vision, pharmacy, clinical quality, and other health and wellness service areas.

Integrated health is a coordinated team-based, person-centered approach designed to bring together all the components that make humans healthy - physical, oral, pharmacy, and behavioral (brain)- focusing on providing comprehensive care to improve outcomes and ensure health equity for all.


This position will focus on oral health and applicants must have subject matter expertise in delivery of oral health services. Experience in working with federally qualified health centers is strongly preferred. Job responsibilities will include leading the Oral Health Strategy for federally qualified health centers through facilitation of oral health work group and leadership meetings, and grant specific work related to integrated oral health, Preference to applicants with strong oral health background and/or licensed/certified dental provider, RDH, or dental assistant.

  • Contribute to the organization’s mission and vision by adhering to the core values and supporting the focus on improving quality and reducing the cost of care at Iowa health centers while promoting health equity for all
  • Serve as a lead for the Iowa PCA’s integrated health strategy work with priority to the assigned focus area(s), including continuing ongoing opportunities for health center staff to connect around integrated health work, creating technical assistance and training opportunities for health centers, and continued assimilation of integrated health as outlined in the Iowa PCA Strategic Plan.
  • Serve as the lead subject matter expert for the assigned focus area(s) including providing technical assistance (TA) and training to participating health centers, coordinating activities with participating health centers, cross-state PCA partners, and HRSA, ensuring all grant objectives are met, participating in all required calls and meetings, and assisting with the creation of progress reports and other documentation as needed.
  • Manage assigned grant program initiatives including coordination of participating health centers and the grantor, ensuring that all grant objectives are met, participating in all required calls and meetings, and creating progress reports and other documentation as needed
  • Facilitate Integrated Health Leadership meetings, Work Groups, Committees, and Learning Cohorts, Project ECHOs, and facilitate/participate in statewide and cross-state Coalitions to discuss innovations and ongoing strategic planning related to Integrated Health and assigned focus areas.
  • Support the eHealth strategy work including ongoing opportunities for health center staff to connect around telemedicine, providing technical assistance and training opportunities for health centers to implement eHealth strategies/solutions and enhance integrated health services through eHealth and telemedicine related to assigned focus area(s).
  • Assist health centers in maximizing the use of their Electronic Health Record (EMR) to support workflows and data collection to support projects and other network goals and initiatives. Develop subject matter expertise for the hosted EHRs related to assigned focus area(s), if applicable.
  • Work with other members of the Transformation Team to provide content, support, and guidance to Iowa PCA, IowaHealth+ and INConcertCare committees and work groups related to Integrated Health and assigned focus area(s) specific content.
  • Assist with coordination, providing content, and facilitating in-person Transformation Collaborative events, which occur quarterly, as needed.
  • Provide training, coaching, and technical assistance to health center staff in integrated health operations, quality measurements, and incorporation and delivery of comprehensive integrated health services into the health center system, with a focus on assigned area(s).
  • Provides technical assistance, relative to the assigned focus area(s), in the development of appropriate policies, protocols, and procedures, including clinical, administrative, and operational policies, which are in compliance with the most current accepted professional standards for health centers across Iowa.
  • Provide support to the development of process improvements to improve operational efficiency and meet the health centers’ strategic goals related to integrated health with a focus on assigned area(s). Assist health centers in creating workflows to optimize care delivery, introduce best practice improvements, and evaluate outcomes. Meet with health center staff as needed to facilitate progress and address barriers
  • Assist with data gathering and analysis in support of integrated health initiatives specific to assigned focus area(s) as needed.
  • Assist with preparing and presenting progress against goals monthly to Boards, Members, Committees, or health center executive teams as directed.
  • Participates in the preparation of grant applications or other funding proposals as needed for additional funding to support this position and other integrated health initiatives
  • Perform other duties and projects as assigned, exhibiting willingness to adapt schedule when requested to get the job done.
  • Participate in and support internal continuous improvement projects.
  • Provide content, support, and guidance to IowaHealth+ and Iowa PCA committees and working groups as needed, taking the lead on select committees and work groups as assigned.

Telecommuting and Travel:

This position is required to be on-site at least two days per work week.

Some travel may be required to community health centers via vehicle or conferences via air travel.



This position has no supervisory responsibilities.

Education and/or Experience:


  • Bachelor’s degree required and Masters in identified area of focus preferred.
  • License to practice the person’s specific profession in the State of Iowa preferred, if applicable.
  • 5+ years of clinical, public health, or administrative/managerial experience in healthcare-related field
  • Administrative, management, performance improvement, quality assurance, grant writing, and budget development experience preferred
  • Practice experience required as it related to identified area of focus
  • Preference given to candidates with experience in Federally Qualified Health Centers (FQHCs) and/or Rural Health Centers (RHCs) or direct experience with Medicaid population.
  • 3+ years of experience in program management strongly preferred
  • 1+ years of experience managing process improvement and change initiatives for health care teams or in other settings - demonstrated ability to motivate and encourage successful process changes in ambulatory care settings, delivering measurable improvements strongly preferred
  • Knowledge of public health principles and practices preferred
  • Certification in process improvement methodologies is a plus but not required.
  • Experience achieving objectives through close collaboration with subject matter experts, external stakeholders, organizational leaders, and peers
  • Ability to manage work simultaneously on a variety of programs with varying deadlines and program responsibilities
  • Excellent oral & written communication skills; able to communicate effectively with organization leadership, health center staff, clinicians, community leaders, and government officials
  • Experience planning and facilitating meetings and group work in a highly collaborative, interdependent environment
  • Experience using data analysis to drive initiatives and/or process redesign in either a consulting role or a supporting role, preferably in healthcare
  • Must be flexible, self-motivated and a team-player
  • Ability to travel around the State of Iowa at least 25% of the time in order to build relationships with health center staff through in-person contact.
  • Intermediate proficiency in MS Office; prior experience with analytics tools.
  • Candidate will be expected to maintain appropriate clinical privileges, if applicable

Language Skills:

Ability to read and interpret documents, operating and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of an organization.


Mathematical Skills:

Ability to calculate figures and amounts such as discounts, interest, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.

Reasoning Ability:

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.


Computer Skills:

To perform this job successfully, an individual should have knowledge of Microsoft Office programs, including Teams, PowerPoint, Excel, as well as Asana.
