Purpose: Define components of the IowaHealth+ Model of Care to do the following:

• Articulate how our network functions, relates to other systems, and produces desired outcomes (roles, responsibilities, etc.)
• Standardize necessary definitions, processes, and components to ensure we can meet expectations (contractual and otherwise)
